If you have an HMO with a PCP
selection, you may select Dr. Bauer or
Dr. Disraeli as your PCP. This will allow
any of us to provide you with Primary
Care services as well as caring for your
urgent needs.

Payment Policy
Payment is expected at the time of service. We accept cash, checks, Visa, and Mastercard.
Our office contracts with most private insurance plans and workers compensation. We will
collect all co-pays, deductibles and co-insurances that are due. Unfortunately, there are
times when an insurance card can not be verified, typically this occurs after hours or on the
weekends. In these circumstances, we will collect 70% of the bill and submit it to insurance.
We will happily refund any amount owed after insurance pays.
